Paracolic Gutter Mass

Either of two grooves formed by the peritoneum and lying respectively lateral to the ascending and descending colons.

The right lateral paracolic gutter runs from the superiolateral aspect of the hepatic flexure of the colon down the lateral. It is the depression between the postero lateral wall of the abdomen and the lateral margins of the ascending and descending colon. These images look quite similar to images of a pseudomyxoma peritonei which was discussed before. Between the outer wall of the colon and back side of the abdominal wall there is an open space known as the paracolic gutter.

Tumors also occur at other sites of fluid stasis including the mesenteric folds right paracolic gutter and subhepatic and subphrenic spaces. Peritoneal carcinomatosis is the most common secondary tumor affecting the peritoneal cavity. The inframesocolic space also contains paracolic gutters which are peritoneal recesses that are inferolateral extensions of their corresponding inframesocolic spaces on the posterior abdominal wall lateral to the ascending and descending colon respectively. The right paracolic gutter is larger than the left and communicates freely with the right subphrenic space.

Paracolic gutters refer to open areas between the wall of the abdomen and the colon. The paracolic spaces gutters are located lateral to the peritoneal reflections of the left and right sides of the colon fig 8a. In a male patient this is a very uncommon diagnosis. Disease may spread from the retroperitoneum which contains the aorta to the peritoneal space along the peritoneal cavity around aortic branches which extend into the mesenteries the so called subperitoneal space 4 5.
